INDUCTEE NOMINATION PROCESS
Candidates for induction into the Ventura County Sports Hall of Fame must be nominated by completing the application form below. This form must be filled out completely for consideration. It is strongly suggested that the person making this submission consult with the nominee or his/her family in gathering complete and accurate information. Submission of electronic documents and photos in support of the nominee is highly encouraged. Deadline for submission is December 1st for the following year.
After they are received, these forms are forwarded to an anonymous Selection Committee, and they will remain active for 5 years from the date of submission. If not selected during this 5-year period, updated applications may be resubmitted.

ATHLETE
Describe scholastic (high school or college) achievements in the nominee’s athletic field – awards won, records established, any recognition by peers, newspapers, etc. If the nominee participated in post-scholastic competition at either amateur or professional levels, please list such teams, contests, awards, etc. with their dates and locations. Include any specific anecdotes or experiences that have been significant to his/her career. COACH
Describe the nominee’s coaching activities in Ventura County only. List sports that s/he coached and years of participation. Include championships, awards won, recognition by peers and media. Please attach electronic copies (PDF or JPEG) of any pertinent newspaper articles, photos, letters of recognition or recommendation, etc.
OFFICIAL
What sports has the nominee officiated? How long has s/he participated? What is the highest level of officiating achieved? List any awards or recognition of his/her excellence by peers. Please attach electronic copies (PDF or JPEG) of any pertinent newspaper articles, photos, letters of recognition or recommendation, etc.
ORGANIZER
How has the nominee contributed to the growth of sports in Ventura County? Did s/he establish or promote youth sports e.g. Little League? Did nominee develop playing fields, stadiums, or athletic facilities? Did s/he institute scholarships or athletic awards? List the nominee’s achievements and the years of participation. Please attach electronic copies (PDF or JPEG) of any pertinent newspaper articles, photos, letters of recognition or recommendation, etc.
MEDIA
How did the nominee contribute to the growth of sports in Ventura County? Was s/he involved in the promotion of professional sports, prep or collegiate athletics? Was the nominee a proponent of any athletic recognition awards, organizations, All Star games, etc. for county athletes? What is his/her resume of sports reporting? Does his/her body of work cover all levels of county athletics? Did the nominee participate in county sports her/himself? List the nominee’s achievements and the time span of his/her activity.
